2.  Supporting documents: Attach all the relevant supporting documents to the OA, such as appointment letters, promotion orders, transfer orders, disciplinary proceedings, and any other documents that substantiate your case.

3.  Affidavit: Prepare an affidavit affirming the truthfulness of the facts mentioned in the OA. The affidavit must be notarized.
